Rename "boehm-gc.h" to "bdw-gc.h"; add to the distribution.

* libguile/Makefile.am (modinclude_HEADERS): Add `bdw-gc.h'.

* libguile/bdw-gc.h: Rename from "boehm-gc.h"; users updated.  Update to
  LGPLv3+.
