renumber VM opcodes

* libguile/vm-i-loader.c:
* libguile/vm-i-scheme.c:
* libguile/vm-i-system.c: Renumber ops. Add a foreign-call op stub.
  Rearrange some ops.

* libguile/_scm.h (SCM_OBJCODE_MINOR_VERSION): Bump
