Remove these unused, unsafe functions. They were publically available
only to C.

struct-ref / struct-set! instead of %fast-slot-ref / %fast-slot-set!
from (oop goops internal).
* module/oop/goops.scm, module/oop/goops/active-slot.scm,
module/oop/goops/composite-slot.scm, module/oop/goops/describe.scm:
Add a copyright line for Érick Gallesio. Mention the STk version
GOOPS was derived from. Remove mentions of the ‘COPYRIGHTS’ file.

Moved ice-9/ and oop/ under module/, with the idea being that we have
only scheme under module/. Adjusted configure.in and Makefile.am
appropriately. Put oop/ at the end of the compilation order.
